Ethics, medication safety, accountability, and stress management are part of my current education plan.
Counseling, psychiatric care, peer support, reflection, and trusted guidance give me structure.
I track each requirement, deadline, submission, test, medication disclosure, and employment limit.
How progress is measured
I separate documents that are ready for my rehabilitation portfolio from requirements that are still in progress.
I want this record to be accurate, not polished to look complete. Each status is backed by the evidence I have.
Organized evidence
“Portfolio ready” means the item is organized as evidence. It does not mean every probation requirement is complete.
Filed as the controlling source for requirements and dates.
Documents what happened, my responsibility, and what I learned.
Adds relevant context without minimizing what happened.
Sets out my treatment, support, prevention, and safer-practice goals.
Keeps APNA materials, certificates, and written reflections together.
Includes peer-support information and ten dated reflections.
Compliance plan
Unfinished items stay visible until they are completed, documented, and submitted through the required channel.
Submit a Prescription Identification Form for each current prescription and the required OTC and PRN list.
Contract with a Board-approved TPA, then follow its daily check-in, testing, and reporting instructions.
Complete the required course package and retain certificates, narratives, and delivery receipts.
Complete the substance-use assessment process and confirm that required results reach the Board.
Continue treatment, peer support, reflective learning, and organized attendance documentation.
Disclose restrictions, confirm duties in writing, and accept only work that fits the agreement.

Raise funds for life-changing care, support and research. Join an Alzheimer's Association event or create a fundraiser to honor someone affected by the disease.

1 Fundraising
Fundraising for the Alzheimer’s Association gave me a chance to support a cause that affects so many individuals and families. It also reminded me that helping others goes beyond the care I provide as a nurse. Raising awareness and encouraging others to get involved are meaningful ways I can give back to my community and support something that matters to me.
Practice commitments
Every job, duty, and schedule must fit my settlement agreement. If I'm unsure, I get written confirmation before I accept it.
Use one non-agency nursing employer at a time and verify every proposed role against the agreement.
Avoid home health, substance-use treatment, Department of Corrections, critical-care, floating, and management assignments.
Maintain no access to controlled substances and obtain written clarification when duties are uncertain.
Keep dated copies, delivery receipts, and written confirmation for every required submission.
A continuing commitment
“I can't change the decision that brought me here. I can choose what I do from this point forward.”
I want the lessons from this period to shape how I care for patients, make decisions, and work with others long after probation ends.
